Assessment Tutorial

The image below shows the assessment screen. Each numbered section is described below. Use the table of contents above to jump to each section.



1 Filters and Search

Here you can filter the sections and questions in the section navigation panel.

Select to see all questions, only the unanswered question, questions that resut in non-conforming findings, or questions with unresolved to-do tasks.

Use the Search field to enter a word in a question or search for a word in observations using the checkbox.

2 Section Navigation

This displays the sections and questions or filtered sections and questions.

Questions in blue text have been answered.

If a section title is blue all of the question in the section have been answered.

The underline question is the current question being asked.

The sections can be expanded and collapsed. A question must be clicked to go to that section and question.

This navigation panel can be collapsed and expanded usign the left and right facing triangle. On smaller screens, this icon button toggles between the main question panel and the navigtion panel.

3 Context

  • This displays the assessment name
  • The question display mode
  • The the current section
  • The access mode of modify, or if shared modify, read-only, or comment-only
  • The number of answered/total scorable questions

4 Client Information

The "prepared for" field is used to associate a client, facility, location, or department with the assessment.

Click to expose the drop-down, selected a client, or add a new client. After adding a new client the client must be selected.

To manage your client list, use the Clients dashboard in the "more..." drop-down.

If a client has been set, the client's information appears at the top of the summary on the reports. The client information is optional.

5 Actions and Status

Click the "report" button to open the report on a new tab

The "clear all"" button removes all answers, observations, flags, and to-do items. The attached images and documents remain.

The list icon links to the assessment dashboard.

The (?) button is found on most screens and brings up the related tutorial file.

6 Question and Tips

This displays the question id, the question, and a question tip. When provided by the author of the program, the tips display helpful information that dosen't appear on the reports.

The question id format is S04Q02, where S is the section and Q is the question in that section. You may notice gaps in the sequence numbering. This is the result of logic rules that may exclude questions or entire sections. For example, if the question, "Are there swimming pools on the property" is answered "No", then an entire section or individul questions about pools might not appear. The question ids are fixed so this results in the gaps.

7 The Answer

The answer to a question is displayed here. This is useful when using the show all mode. If a question has not been answered "none" is displayed.

8 Answer Options and
9 Action Buttons

Respond to the questions here. A question might require a single selection, selecting all options that apply, entering text, or entering a number. Refer to the four image as follows:

  1. On the top an answer option has not been selected. Tho only action button showing is skip forward ">>" to the next question.
  2. The second image show that an answer option has been selected and the proceed button is displayed to save the answer and advance to the next question.
  3. The third image displays how an answered question appears when you return to an answered question. The reset action will reset this question to an unanswered state.
  4. The last shows when a different answer option is select, the Update action is shown. Click the Update button to save the changed answer and then >> to advance to the next question.

NOTES: An informational item (question) simply displays the Proceed button. To move to a previous question or jump to questions in other sections use either the "Show all" mode or tha navigation panel described in Section 2 of this tutorial.

10 Supplemental Information

Switch between the tabs to reveiw reference information, add observations, attach photos and documents, or view the history of response changes.

On a tablet, photos can be directly taken and attached to the assessment. Click on the attachments tab to access the Add attachment button.

Note: GapCross is easier to use on a tablet, laptop, or desktop however a mobile phone can be used to capture photos at the same time the assessment is conducted on a large device.

There are four type of observations that can be added using the green icon buttons.

  • Comments that are simple text observations.
  • Flags that include a flag-type and a comment. Four flag types are built into GapCross: attention, corrective action, preventive action, and deviation. Deviations are primarily used for procedural applications to record deviation details from the standard procedural steps.
  • To-do items are used to record tasks that need to be completed at a future time. When the task is complete click the checkbox. An assessment can't be transitioned to a permanent record until all to-dos are completed. Use the "To-do" filter described in section 1 of thhi tutorial for locating open to-do tasks.
  • Hidden comments are notes that do not show up on the reports. However, if the assessment is share with another user account that person will be able to see the notes so approprate wording is always recommended.

All of these entries have a reply and a trash button that appears on the right of the observation. The reply allows follow on comments and trash deletes the entry. Deleted entries can be displayed by clicking the display filter checkboxess at the bottom right. Deleted items appear with strikeout text.

To-do items have a box to check when the to-do item is completed.

Observations and replies can be modified by cliking the text.

11 Next Question

This is the next question when the "Show all" mode is used. Previous questions appear above as well. Any question can be click to jump to the question.

If an answer has not been recorded for the current question jumping to a new question will activate a warning message with options to cancel the actin of jumping elsewhere or abandoning the changes and proceeding to the selected question.

To jump to a question in a different section use the section navigation panel on the left.
